Esketamine and PTSD: What to Know About This Groundbreaking, Effective Therapy

What Is Esketamine Therapy for PTSD?

You’ve likely heard of ketamine, which is made up of two molecules called R-ketamine and S-ketamine. Ketamine treatments are IV infusions.

Esketamine, on the other hand, is a refined version of ketamine, as it’s only S-ketamine. It’s administered as an intranasal spray known as SPRAVATO®, which has been shown to be an effective treatment for both PTSD and depression.

How SPRAVATO® Works

SPRAVATO® works differently than traditional antidepressants, which target serotonin pathways and typically take weeks or even months to offer relief. SPRAVATO®, however, doesn’t target serotonin—it influences the brain’s glutamate system, the most common neurotransmitter in the brain.

By enhancing that system, SPRAVATO® helps form new neural connections, allowing the brain to “reboot.” Many have been able to improve their mood, reduce the emotional weight of PTSD, and process traumatic memories after just a few treatment sessions.

What to Expect from Esketamine (SPRAVATO®) Therapy for PTSD

Here at SD Wellness Group, SPRAVATO® treatments are self-administered in our comfortable outpatient setting under the supervision of trained medical staff. It’s natural to feel anxious about exploring new therapy treatments, but knowing what to expect can help you feel more at ease.

Here’s a general overview of esketamine therapy for PTSD:

Session Length

The total session time is about two hours. While the spray is quick and easy to administer, there is an observation period following treatment. We’ll monitor your vital signs and watch for side effects (rare but can include dizziness or slight nausea) to ensure your overall well-being.

Please note that you’ll need to arrange transportation to and from your session, as you may not drive immediately after receiving SPRAVATO®.

Administration

Your clinician will walk you through how to administer SPRAVATO® and answer any questions you have. Each nasal spray contains two sprays—one for each nostril. Before starting, you’ll blow your nose to clear your nostrils.

Then, you’ll hold the spray the way your clinician has shown you, tilt your head about 45 degrees, and use your other hand to close the opposite nostril. You’ll breathe in while pushing the plunger down, releasing the spray into your nose. After spraying, sniff gently to ensure the medication stays inside your nose.

You’ll then repeat the process with the other nostril.

Session Frequency

While everyone is different, many people begin with treatments twice a week for four weeks, followed by a gradual tapering off based on progress. A total of eight to ten sessions is not uncommon during the initial treatment phase. For sustained results, ongoing maintenance sessions may be recommended.

Immediate and Long-Term Effects

Many experience symptom relief within hours to days after their first treatment. Depending on how you respond to treatment, you may experience a more gradual improvement over a series of sessions.

Who Can Benefit From Esketamine to Treat PTSD?

  • Those with treatment-resistant PTSD: If you’ve tried multiple therapies and antidepressants but have yet to achieve lasting relief, esketamine could be right for you.
  • Those who haven’t responded well to traditional therapies: Conventional, serotonin-based treatments and medications don’t work for everyone. You may be a good candidate for esketamine if traditional therapies have let you down.
  • Those struggling with severe depression related to PTSD: If your post-traumatic stress disorder is compounded by severe depression, esketamine may offer the rapid relief you need.
