What to Expect
Starting something new can feel overwhelming. Because knowing what to expect can help ease your nerves, here’s an overview of our mental health and substance use treatment process.
- Initial setup: You start by creating a patient portal, completing insurance and intake forms.
- Consultation: Whether this is your first time seeking support or your tenth, we’re here to listen, not judge. During this initial evaluation, we’ll discuss your challenges and previous treatments.
- Personalized care plan: No two people are the same, and neither are their paths to recovery. We’ll tailor a care plan that fits you—not the other way around.
- Long-term guidance: Because healing doesn’t happen overnight, we offer ongoing care, resources, and a community that will continue to support you as you grow.
How to Get Started
Initiate Patient Portal
Our secure patient portal allows you to message your provider, request medication refills, and more. To get started, head to our patient portal, answer a few questions, and upload your insurance information.
Register & Complete Forms
Once your insurance information has been verified, we’ll send you a welcome email with instructions on how to officially register your patient portal. You’ll then need to complete the intake forms found in the “Forms” tab.
Schedule Evaluation
Once all forms are complete, give us a call at (619) 320-8711 to schedule an initial mental health consultation. Please note that consultations can only be scheduled if all intake forms have been completed.
*If you’re active duty military or utilizing TriWest insurance, we need a referral faxed to our office at (619) 393-0818.
Printable Forms for Your Convenience
If you prefer to fill out your intake forms by hand instead of online, you’re welcome to print them out. Complete them at your convenience, and email your completed forms to info@sdwellnessgroup.com.
