If you prefer to fill out your intake forms by hand instead of digitally, please print, complete, and email the following forms to info@sdwellnessgroup.com.
Please also include a front and back photo of your driver’s license or ID. If you plan on using insurance, we’ll need a front and back photo of your insurance card as well.
